DMV/VERIFI Message #134

Attention: Dealer Plate Issuance Participants.

All applications must be completed in their entirety, including registrant(s) and owner(s) signatures, AND all paperwork must be submitted for processing within 5 calendar days of issuance in VERIFI.

Applications submitted to DMV must be accompanied by all required documents, specific to the type of transaction, as indicated below.
 

➢ PERMANENT REGISTRATIONS MV-465 Submission to local DMV Office/Processing Center
A permanent registration and title will not be issued without the following documents:

• Original/Completed MV-82DEAL application, signed by the registrant(s) and owner(s), when applicable. (see Section 3.2)
• VERIFI Transaction Receipt, along with any other transfer documents showing continuity.
• Original proof of ownership.
• NYS bar-coded insurance card in the name of the registrant(s), as applicable.
• Copies of Proof of Identity/date of birth or Proof of Corporation - for both the registrant(s) and owner(s).
• (Plate transfer only) A copy of the current valid registration.
• Supporting documents, as necessary. (i.e., POA form MV-93, Dealer Authorization statement, etc.)

➢ INTERSTATE IN-TRANSIT PERMITS (ITP) MV-465ITP Submission to DMV Central Office
An Interstate In-Transit Permit will not be processed without the following documents:

• Original pre-numbered tear-strip from the In-Transit Permit Plate.
• Copies the completed parts 1 and 2 of the In-Transit Permit issued to the customer.
• Original/Completed MV-82ITP application, signed by the registrant(s) and owner(s), when applicable. (see Section 4.3)
• Copy of the Certificate of Sale (MV-50/eMV-50), along with any other transfer documents showing continuity.
• Copy of the proof of ownership (front and back).
• Copy of proof of financial security: NYS insurance card or Insurance letter/binder from an out-of-state insurance company in the name of the registrant(s).
  • Must contain registrant’s name and address; Insurance Company’s name and address; Policy number; effective date; and Vehicle info (Year, Make, and VIN).
• Proof of Identity/date of birth or Proof of Corporation - for both the registrant(s) and owner(s).
• Supporting documents, as necessary. (i.e., POA form MV-93, Dealer Authorization statement, etc.)

Remember: Voided ITPs must be submitted on a separate transmittal, from those to be processed. 
Important: Applications for stretch limousines CANNOT be processed at a DMV Office.
